There is one game locally that I know of (home game) that does a same / same blind amount. $5/$5 and they are treated simply as "blinds", versus small and big. Of course both have options, etc.
The only other time I've ran across this recently was in a session at the MGM / LV, where they have a $5/5 NL game. Very soft game. I didn't have a great session in it, but some of the play was ridiculous.
Those are the only two that I know of.
Three blinds is more common. I know of several games that have a button post with relationship to the blind posts. |

Captn | The El Dorado in Reno has a NL game. One of my favs around.
It's a $2/$3/$5 with a $10 bring in. So it's 2 on the button, 3 SB and 5 BB, then 10 to go.
There is a local card room that has a very very beatable 9/18 game which is $3/$3/$9 Button, SB and BB respectively.
Another NL game around is a $1/2/5, the $1 on the button is dropped for the jack pot.
Basically, it's just a post on the button. Can be used for a drop or as part of the blinds, and or added to the pot. One thing that it does do, which also (can) creates more action, is it prevents the blinds from chopping if the table folds around to them. |

Like I said, one of my favs. Reno is about a 2 hour drive for me, so I make it there about once a month to play in it, at times, more. They also don't cap the buy in, so there can be some pretty strong stacks on the table. That being said, the game is very action packed. I've actually adopted that format (other than the button post) with a $2-SB/$5-BB/$10 Bring in format for my home game when I get the right / interested players. |

I'm trying to put together a more serious (or "aggressive" or "experienced") night where we would just play hold'em (my social game is dealer's choice) for slightly higher stakes. Not sure if I know enough people who are that serious about it.
Although I am expanding my poker circle...
