With cash games players can buy in (as long as you don't exceed the maximum number of players for a table), or buy back if they bust out or get below a certain value in chips.
The buy-in maximum for my home cash games is 10 x the big blind. You can never buy-in for more than that but you can amass more chips by winning pots. You can buy in for less, but of course if you have the nuts and someone is betting in to you, you can only ever double up the chips you have.
The blinds never increase because you are not trying to get down to a single player, in fact quite the opposite.
we still use the sb and the bb and it rotates as normal.. the value of those never increase... unless we decide to change the stakes then its a one time thing.. we normally play $1/2 but have been known to on occasion raise them to $2/$4 or $4/$8!
The only place I've ever played a cash game that did it differently than mentioned above was at Luxor. They had 2 small blinds and 1 big blind, which was really, really annoying. That was a couple years back and may have changed by now though. Hopefully.
