Called the Flower Mold. Supposedly, the owner is living in FL. Supposedly has the old records as well. This is a fairly old mold. Here is another example:

While the "Mustang Lounge" is unknown, it seems that this chip goes back to a time that real gold was used for the hot-stamp!
