Public card room rules, etiquette, customs and expectations

Wikipedia has this excellent article on public card room rules and some of the state variations that often affect betting
Public cardroom rules (poker) - Wikipedia, the free encyclopedia . As with any Wikipedia source though this information could lack accuracy. I'd like to use this thread to expand on card room rules, etiquette, etc. Some ideas for inclusion are found in the Wiki article sited:
waiting lists
obtaining chips
whether cash plays
rake or hourly charges

Others are:
games spread
mandatory move tables
tipping - dealer, tourney staff, chip runner
professional dealers
other card room staff
tournament structures
etiquette
state limitations such as only limit games being spread in Colorado

Anything else you guys think might be good to assemble? I know that the first time I set foot in a public card room I was scared to death and had no clue how to proceed. Might be nice to have a thread here with some good general information on what to expect in public card rooms.

California pro Steve Badger's playwinningpoker.com site has a decent article on card room etiquette here Poker Etiquette - Crybabies, Whiners and Losers . Nothing to do with etiquette, but the article has a great line in it "Watching Phil Ivey play Omaha is like watching God print money".

He's also quoted in this San Francisco Chronicle article on poker protocol in public card rooms POKER PROTOCOL / Cardroom manners / Former pro offers etiquette advice to those playing among strangers at casinos .

Nice thread...I was pretty well out of my element the first couple times I played in a live cardroom.
