John Vorhaus identifies this game exactly as you have in his home game primer
Poker Night: Winning at Home, the Casino and Beyond (which I highly recommend, BTW). He mentions that he's seen it at a smaller club in Ventura, CA called the Player's Club.
If you think five-card Omaha is wild, try mixing it with tic-tac-toe, especially as JV suggests (he calls it "Bughouse" in the book, and adds in the option to use all five cards in your hand instead of making your hand from two in your hand and three on board; I call this game "Tic-Tac-Dough," in honor of one of my favorite game shows)
