Also, if they are used, are players required to purchase another when after they rebuy (after busting out)? It seems like it would be necessary, but I wasn't sure.

We use bounties all the time in our cash games. We call them chips

No go on the bounty chip. Don't see the point. But I'm a bit of a poker puritan. Set me down with a good dealer, adequate cards, decent chips, and nothing wild, and I'm ready to go.
