5Star and discountcasinogear offer them with hotstamping, for pretty good prices, but not custom logos. vegassuppliesandgifts offers custom but is by far the most expensive. I could swear that 5Star USED to offer custom hotstamping for like an $80 set-up fee.

Anyone have any experience with getting custom hotstamped chips and would like to share their knowledge or experiences with me?
